Cleara commercializes one of the most-cited proofs-of-concept in the entire senescence field: founder Peter de Keizer's FOXO4-DRI peptide, whose 2017 demonstration that disrupting the FOXO4–p53 interaction can selectively drive senescent ("scarred") cells into apoptosis — restoring fitness in old mice — is a landmark result that put targeted senolysis on the map. Spun out of University Medical Center Utrecht, Cleara has advanced beyond the original peptide to two optimized D-amino-acid lead candidates, CL04183 and CL04177, engineered for improved stability and niche-directed activity and each paired with biomarkers for patient selection — a precision-medicine framing that could distinguish it from blunt small-molecule senolytics. The preclinical package is substantive: the leads potently kill scarred cancer cells in 2D culture and 3D organoids and strongly reduce metastatic burden in mouse models of metastatic colon and triple-negative breast cancer, with favorable pharmacokinetics and tissue distribution. That oncology angle is strategically smart — targeting therapy-induced senescent cells in late-stage cancer offers a faster, better-reimbursed regulatory path than a healthspan indication, while validating the platform for chronic age-related disease later. Backed since inception by Apollo Health Ventures, Cleara holds genuine scientific pedigree in a hallmark of aging that pharma increasingly takes seriously.
