Eko Health has quietly assembled the largest AI-enabled auscultation franchise in medicine: more than 750,000 of its digital stethoscopes are in clinical hands, the company says over 500,000 healthcare professionals use its platform, and it has earned nine FDA clearances spanning hardware and algorithms — including murmur and structural-heart-disease detection, atrial fibrillation, and, via a Mayo Clinic-built algorithm, low ejection fraction, allowing a primary-care clinician to screen for hidden heart failure during a routine exam with the instrument already around their neck. That last clearance is the business-model unlock: heart failure is enormously expensive precisely because it is caught late, and a 15-second, FDA-cleared point-of-care screen embedded in the standard physical exam is the cheapest imaginable detection funnel — a workflow story no echo cart, ECG patch, or consumer wearable can match. The moat compounds across layers: the CORE 500 flagship pairs 40x amplification, noise cancellation, and 3-lead ECG with a recurring software subscription; the multi-year 3M Littmann relationship put Eko electronics inside the most trusted stethoscope brand in medicine; Mayo Clinic is both algorithm partner and investor; and every exam recorded enriches a proprietary heart-lung sound dataset that trains the next algorithm generation. Capitalization is adequate and disciplined — roughly $125–128M raised, most recently a $41M Series D in July 2024 from ARTIS Ventures, NTTVC, Highland, and Questa. As health systems hunt for AI with demonstrable ROI and reimbursement pathways for AI-assisted screening mature, Eko owns the rare combination of installed base, regulatory assets, and clinical-brand trust — the position from which point-of-care screening platforms get built, or bought at a strategic premium.

Eko's installed base is larger than its business model has yet proven: selling 750,000 stethoscopes is hardware revenue with long replacement cycles, and the company's platform economics depend on converting clinicians — and more importantly their employers — into paying AI-subscription customers, an attach motion whose success remains undisclosed and whose buyer (the health system) is the slowest, most budget-constrained purchaser in the economy. The reimbursement pathway that would supercharge adoption — dedicated payment for AI-assisted auscultation screening — remains immature, leaving Eko's algorithms as unfunded clinical virtue for many practices; without a billing code, "detects disease earlier" competes with every other unbudgeted good idea in medicine. The structural threat is that auscultation itself is a fading art being leapfrogged: handheld point-of-care ultrasound (Butterfly, GE Vscan, EchoNous) gives clinicians direct cardiac visualization at falling prices, AI-ECG platforms like AliveCor attack arrhythmia screening from a different sensor, and cheap patch monitors capture the rhythm data longitudinally that a stethoscope samples for seconds — meaning Eko's screening wedge could be outflanked by modalities with richer signal. Its algorithm partnership with Mayo Clinic cuts both ways, sharing economics and credit for the most differentiated asset. Competition in digital stethoscopes has commoditized the hardware layer, so the defensible value is all in the FDA-cleared software — a thin layer to support a company of 201–500 people if subscriptions stall. And with no raise disclosed since mid-2024 and no stated path to profitability, Eko faces the medtech squeeze: too clinical for consumer scale, too device-bound for software multiples, and dependent on health-system sales cycles that outlast funding runways. The base case may be a solid, modest diagnostics company — not the platform its dataset narrative implies.
